Care Coordinator

Manage a full patient caseload through app chat, writing clear, warm, professional messages.
Meet response-time and quality metrics, including responding within 4 hours during scheduled hours.
Anticipate patient questions and resolve issues in as few touchpoints as possible.
Coordinate with pharmacies and labs by phone to troubleshoot prescription issues, insurance hiccups, and delivery problems.
Connect with dermatology providers as needed to keep care plans on track.
Keep meticulous patient records.
Monitor patient progress through ongoing messaging and troubleshoot obstacles.
Share feedback on workflows and patient trends to help improve care delivery.
