Requirements
- Have at least 1 year of experience working in Health Care IPAs.
- Have at least 1 year of experience entering referrals and prior authorizations.
- Hold a high school diploma or GED, or have a bachelor’s degree or four additional years of experience in lieu of education.
- Be bilingual in English and Spanish.
- Have knowledge of Managed Care Plans and Medi-Cal.
- Have basic computer skills, including Microsoft Outlook, Excel, and Word, and a typing speed of 25 WPM.
- Have effective problem-solving, organizational, and time-management skills and the ability to work in a fast-paced environment.
- Communicate positively, professionally, and effectively with others; collaborate and provide leadership or instruction as needed.
- Preferred: MBA, Medical Assistant training, and Medical Terminology training.
Responsibilities
- Contact members by phone to assist with referrals, authorizations, HHC and DME needs, medication refills, provider appointments, and follow-ups.
- Create cases and tasks and complete assessments for hospital and SNF discharges in the Case Management module.
- Complete tasks assigned by the nurse and document work as appropriate.
- Work with the Case Manager to engage and manage a panel of SNP members.
- Monitor new alerts and update the Case Manager about changes in condition, admissions, discharges, or new diagnoses.
- Build relationships with members, earn their trust, and act as a patient advocate.
- Escalate concerns about non-compliance or changes in condition to the nurse.
- Maintain and update member records; request and upload medical records from providers and facilities.
- Attend Interdisciplinary Team Meetings and participate in member presentations as appropriate.
