- Fluent in English (written and verbal); bilingual in English and Spanish is a huge plus.
- Experience
- A minimum of three years of experience helping people manage their health, ideally in a clinical setting and/or a remote health coaching setting
Requirements
- Have a minimum of three years of experience helping people manage their health.
- Hold a degree in a health-related field.
- Have experience counseling or educating patients about disease management in a medical setting.
- Be fluent in written and spoken English.
- Have some experience in a clinical setting.
- Have some knowledge of cardiac rehabilitation.
- Have experience working with an elderly patient population.
- Have virtual care experience.
- Be familiar with cardiac conditions, CHF, or other chronic conditions.
- Have knowledge of recent nutrition and exercise trends.
- Be available for a Saturday or Sunday shift from 8am to 2pm ET.
Responsibilities
- Conduct initial assessments for patients enrolling in the virtual cardiac recovery program.
- Educate patients on program benefits and risk factor modification.
- Coach individuals through chat messaging and video sessions.
- Help patients problem-solve and support their current and evolving goals.
- Adapt in-clinic best practices to a remote-care delivery model.
- Complete individualized treatment plans, including nutrition, psychosocial, fitness, and co-morbidities assessments.
- Translate evidence-based chronic condition management practices into a relatable and empowering approach.
- Motivate and encourage patients during initial visits.
- Review patients’ medical histories and support goal setting using motivational interviewing.
