Requirements
- Have a BA or BS degree in a related field, or substitute four or more years of related experience.
- Have at least two years of experience working with individuals with disabilities and/or senior citizens.
- Demonstrate strong skills with Microsoft Office applications.
- Be able to manage your own schedule and work independently with minimal supervision.
- Demonstrate client service, problem-solving, and a consultancy mindset.
- Communicate effectively orally and in writing and interact with clients and different levels of management.
- Use sound judgment, recognize issues, identify solutions, and seek guidance when needed.
- Be able to follow, critically evaluate, and improve current processes.
- Knowledge of Medicaid services is preferred.
- Bilingual proficiency in English and Spanish is preferred.
Responsibilities
- Manage a workload of up to 125 program participants and maintain required monthly telephone contact.
- Respond to participant inquiries and resolve participant issues.
- Provide practical skills training on locating community resources, hiring and supervising employees, purchasing goods and services, and managing monthly budgets.
- Assist participants with enrollment in the California Self-Determination Program and completion of required paperwork.
- Help participants identify services and supports, develop spending plans, and schedule employees within their budgets.
- Assist direct care workers with paperwork required for employment by participants.
- Monitor participant health and safety through monthly phone contact.
- Maintain progress notes in accordance with NASW standards and report and respond to complaints and grievances.
- Act as a mandated reporter for allegations of abuse, neglect, and exploitation.
