Requirements
- Hold an active, unrestricted RN or LPN/LVN license in a compact state.
- Have 3+ years of clinical nursing experience in bedside care, Primary Care, Med/Surg, or Home Health.
- Have experience providing high-volume patient care in a fast-paced virtual or telehealth clinic environment.
- Be proficient with EHR systems and telehealth or teleconferencing platforms.
- Maintain a dedicated, private home workspace suitable for confidential patient interactions.
- Have reliable internet connectivity and an ethernet connection from the computer directly to the router, meeting minimum speeds of 100 MBPS download and 50 MBPS upload.
- Use a Windows 10 or later or macOS 14 Sonoma or later computer; Chromebooks, Linux machines, and smartphones are not permitted.
- Use a computer with an Intel Core i5 or AMD equivalent processor on Windows, or Apple Silicon on Mac, and 16 GB RAM.
- Use wired headphones and a webcam for video calls.
- Use Google Chrome with Amazon Workspaces.
- Be available Monday through Friday, 10:00 AM to 6:00 PM EST, one Saturday per month from 10 AM to 4 PM EST, and rotating holidays.
- Preferred: experience with Medicare Annual Wellness Visits, knowledge of CMS documentation and preventive care guidelines, and experience across multiple virtual care programs and platforms.
Responsibilities
- Prepare comprehensive Annual Wellness Visit charts to support provider efficiency and CMS compliance.
- Review medical, surgical, and family histories with patients during face-to-face virtual visits.
- Review medication lists and document adherence.
- Review preventive screenings, immunizations, and cognitive, functional, and depression assessments.
- Identify overdue screenings, risk factors, and care gaps before visits.
- Communicate concise patient summaries and key findings to providers before each Annual Wellness Visit.
- Coordinate with patients to obtain missing or clarified information.
- Maintain accurate, timely, confidential patient records that meet Medicare, CMS, and organizational requirements.
- Use CT360, RXNT, Zoom, and telehealth platforms to support care and documentation.
- Support clinical quality, patient engagement, and satisfaction outcomes.
