Requirements
- Active, unrestricted Registered Nurse (RN) license in a compact state.
- Minimum 3 years of clinical nursing experience in bedside, Primary Care, Med/Surge, or Home Health.
- Experience working in a telehealth, virtual care, or remote environment.
- Experience with Chronic Care Management (CCM) or Remote Patient Monitoring (RPM).
- Proficiency with EHR systems and telehealth platforms.
- Strong clinical assessment and care coordination skills.
- Excellent verbal and written communication skills.
- Ability to work independently in a remote environment.
- Dedicated, private home workspace.
- Meet specific technical requirements including high-speed internet (100 Mbps download/50 Mbps upload) with ethernet connection and compatible Windows/macOS hardware.
Responsibilities
- Prepare comprehensive AWV charts to support provider efficiency and CMS compliance.
- Conduct comprehensive nursing assessments via Zoom to address physical, functional, and psychosocial needs.
- Develop, implement, and monitor individualized care plans for programs including CCM, RPM, RTM, and APCM.
- Engage patients and caregivers in care planning and adherence using motivational interviewing techniques.
- Identify and close care gaps such as preventive screenings, labs, medications, and follow-ups.
- Coordinate care across multiple services and providers to ensure continuity and quality.
- Maintain accurate, timely, and confidential patient records in EHR systems.
