Requirements
- Active, unrestricted RN license.
- At least 3 years of people leadership experience managing supervisors or team leads in clinical or production settings.
- Experience in virtual care, telehealth, care management, or population health operations.
- Proficiency with EHR, practice management, telehealth platforms, and CRM systems.
- Ability to manage concurrent workflows and caseload-based panel models.
- Strong data fluency with experience building and interpreting operational reports and Excel analyses.
- Understanding of HIPAA, patient privacy, and healthcare compliance.
- Availability to support 24/7 operations, including rotating weekend and holiday coverage.
- Bachelor of Science in Nursing or related degree preferred.
- Experience with CCM, RPM, RTM, or health plan programs like Medicare Advantage preferred.
Responsibilities
- Lead daily care navigation operations, including case assignment, prioritization, and resolution.
- Enforce workflow standards for clinical tasks, results, provider order follow-up, and referrals.
- Monitor queue health and reallocate work to maintain throughput and turnaround-time standards.
- Manage 24/7 nursing coverage including shift handoffs, rotations, and staffing plans.
- Oversee longitudinal chronic care and remote therapeutic monitoring operations.
- Develop and coach supervisors and clinical leads in performance management and documentation.
- Collaborate with quality and compliance teams to ensure HIPAA and health plan contractual adherence.
