Job Details

Active, unrestricted Registered Nurse (RN) license in a compact state.
Minimum 3 years of clinical nursing experience (bedside, Primary Care, Med/Surge, or Home Health).
Proven experience in a telehealth, virtual care setting, or remote work environment.
Experience in one or more programs: Chronic Care Management (CCM), Remote Patient Monitoring (RPM), or care management.
Proficiency with EHR systems and telehealth platforms.
Strong clinical assessment, documentation, and care coordination skills.
Excellent verbal and written communication skills.
Ability to work independently in a remote environment.
Dedicated, private home workspace.
Reliable internet connectivity with 100 Mbps download and 50 Mbps upload speeds (Ethernet connection required).
Windows 10+ or macOS 14+ computer with at least 16GB RAM and i5/AMD equivalent or Apple Silicon processor.
Wired headphones and a webcam.

Prepare comprehensive Annual Wellness Visit (AWV) charts to support provider efficiency and CMS compliance.
Conduct face-to-face virtual visits via Zoom to review medical histories, medications, and health screenings.
Manage telephonic and portal-based care for patients with chronic, behavioral, and socio-economic comorbidities.
Develop, implement, and monitor individualized care plans for CCM, RPM, RTM, and APCM programs.
Identify and close care gaps related to screenings, labs, medications, and follow-up appointments.
Coordinate care across multiple services and providers to ensure continuity and quality.
Maintain accurate patient records within EHR systems like CT360 and RXNT.
