Requirements
- Bachelor of Science Degree in Nursing (BSN).
- Compact unrestricted registered nurse (RN) license.
- Licensure in Rhode Island (or willing to obtain).
- Minimum 3 years of nursing experience, including 2 years in oncology nursing.
- Proficiency with technology including Slack, Zoom, Google Suite, and EHR systems.
- Basic Life Support (BLS) certification.
- Compliance with company employee vaccination and testing policy.
- Strong interpersonal skills for establishing rapport with diverse populations.
- Ability to prioritize and manage urgent clinical needs effectively.
- Oncology-related certifications (OCN, AOCN, AOCNS, or CCM) strongly preferred.
- Excellent organizational and multitasking skills for fast-paced environments.
Responsibilities
- Provide clinical consultative support via phone, email, or video and handle urgent clinical escalations.
- Triage inbound reported symptoms and Symptom Survey responses, escalating to a Nurse Practitioner as required.
- Proactively identify and address patient needs, including care coordination, nutrition, discharge planning, and referrals.
- Educate patients on diagnoses, Daymark care offerings, and care plan next steps.
- Build trusting relationships with patients and care networks using empathy and active listening.
- Coordinate with integrated care team members including medical directors, nurse practitioners, and social workers.
- Collaborate with non-clinical health partners to address social determinants of health.
- Adhere to company documentation standards and operational procedures.
