Requirements
- Active, unrestricted RN license in at least one U.S. state (Multistate/Compact license strongly preferred).
- 3+ years of RN experience in care management, case management, transitions of care, home health, or primary care.
- Demonstrated ability to build trust with older adults.
- Strong clinical judgment with the ability to identify when to escalate issues.
- Proficiency with EHRs, care management platforms, and phone/video communication tools.
- Self-directed ability to manage own member load and schedule.
- Access to a reliable computer, high-speed internet, and a private, HIPAA-appropriate workspace.
- Experience with Medicare or older adult populations (preferred).
- Case management certification (CCM) (preferred).
- Telehealth or telephonic nursing experience (preferred).
- Bilingual skills, especially Spanish (preferred).
Responsibilities
- Conduct phone and video assessments of health status, medications, and social determinants of health needs.
- Develop and update member-centered care plans with clear goals.
- Complete medication reviews to identify adherence barriers or interactions.
- Coordinate care across primary providers, specialists, pharmacies, and home health agencies.
- Support members through transitions of care, including post-hospital follow-up.
- Connect members to community resources for food, transportation, and housing.
- Reinforce provider treatment plans and help members prepare for appointments.
- Recognize clinical red flags and escalate urgent concerns to providers.
