- Able to work independently in a remote environment
- Private, quiet workspace where patient conversations cannot be overheard
- Reliable high-speed internet
Responsibilities
- Conduct Medicare Annual Wellness Visit screening questionnaires with patients by phone
- Make outbound calls to scheduled and eligible patients
- Document patient responses accurately in the EMR during the call
- Assist eligible patients with scheduling their wellness screening
- Communicate clearly and professionally with patients and clinic staff
- Maintain complete, accurate, and compliant documentation
