Requirements

- Residence in the US.

- Basic knowledge of chronic health conditions and their management.
- Highly adaptable and comfortable working across different medical practices and teams.
- Reliable, flexible, and comfortable working in a float pool environment.
- Comfortable using technology for documentation and communication.
- Strong attention to detail and organizational skills.
- Ability to work independently and as part of a team.
Responsibilities
- Regularly engage with patients and track their adherence to their care plans.
- Closely monitor patients' device readings and investigate any out-of-bounds values.
- Maintain meticulous records of patient interactions and program details.
- Stay up to date on the latest regulations and compliance standards for CCM/RPM services.
- Ensure all patient enrollments adhere to legal and ethical standards.
- Monitor and track care performance, striving to meet or exceed goals.
- Support patients across multiple medical practices, adapting to different workflows, teams, and patient populations.
- Transition seamlessly between practices based on operational needs while maintaining consistent care quality.
