- Must have a valid state driver's license.
- Maintain personal vehicle liability insurance meeting minimum requirements.
- Must have a private home office with a locked door for PHI/HIPAA compliance.
- Home internet with minimum 25 Mbps download and 10 Mbps upload speeds.
Responsibilities
- Evaluate member needs and requirements to achieve and maintain optimal wellness.
- Facilitate interaction with resources appropriate for member care and well-being.
- Identify and resolve barriers that hinder effective care.
- Monitor patient progress using assessments, data, and member conversations.
- Manage active care planning and coordination of services.
- Maintain patient care records and documentation.
