Requirements
- 1+ years of member-facing healthcare experience (e.g., medical clinic, care coordination, or telehealth).
- Availability to work Monday–Friday, 9am–6pm EST.
- Located in Nova Scotia, Canada.
- Proficient with EHR systems, CRMs, and digital communication tools.
- Demonstrated ability to build trust with older adult populations.
- Understanding of care coordination principles and chronic condition support.
- Commitment to compliance procedures and documentation accuracy.
- Ability to work fluidly across multiple platforms simultaneously.
- Adaptability to shifting program processes and team assignments.
Responsibilities
- Conduct outbound enrollment outreach to prospective members using a structured referral queue.
- Perform member-facing welcome visits to explain program support and set expectations.
- Coordinate care gaps by liaising with providers, pharmacies, and specialists.
- Facilitate access to community resources, including transportation, meals, and medication assistance programs.
- Complete and document member interactions and SDOH screenings in real-time.
- Execute daily tasks across enrollment, welcome visits, and care coordination as assigned by the team lead.
