Responsibilities
- Schedule work assignments based on Home Care Worker training and experience.
- Document all communications accurately in HHA Exchange.
- Communicate effectively with patients, families, contracts, clinicians, and aides regarding issues or changes.
- Manage overtime and travel time requests.
- Utilize Smart Map and other tools to locate aides, provide directions, and check availability.
- Complete daily, weekly, and monthly reports as required.
- Respond promptly to emails, voicemails, and faxes.
- Manage mutual and linked cases and complete incident reports.
