Job Details

7+ years of growth leadership experience in hospice, house calls, home health, assisted living, long-term care, or skilled nursing facilities.
Proven track record of recruiting, training, and managing growth teams.
Experience in performance management, coaching, and professional development of staff.
Demonstrated success building relationships with healthcare executives, medical directors, and facility administrators.
Background in post-acute care settings and understanding of referral-based business models.
Deep knowledge of house calls or hospice service delivery models and market dynamics.
Understanding of Medicare, Medicaid, and managed care reimbursement for home-based services.
Knowledge of healthcare regulations affecting hospice or home-based medical practices.
Bachelor's degree in healthcare administration, business, or a related field; MBA or advanced degree preferred.

Recruit, hire, and onboard growth professionals (community liaisons) to build referral partnerships.
Develop comprehensive training programs for growth staff on services and methodologies.
Implement performance management systems including goal setting, coaching, and reviews.
Build strategic relationships with regional health systems, medical groups, and enterprise clients.
Cultivate partnerships with Medicare Advantage plans, Medicaid managed care, and commercial insurance providers.
Oversee growth reporting and analytics, utilizing KPIs to track pipeline health and conversion rates.
Drive achievement of quarterly and annual revenue targets.
Partner with Clinical Operations to ensure alignment between growth commitments and service delivery.
