Requirements
- Active, unrestricted RN license with multistate privilege under the Nurse Licensure Compact (NLC).
- 2+ years of clinical experience involving medication management, care coordination, or utilization review.
- Strong working knowledge of common drug classes and clinical interaction risks.
- Proven track record of high-volume, outcome-driven phone outreach.
- Exceptional attention to detail and documentation discipline.
- Comfortable working within a structured case management system and across multiple EHRs.
- Demonstrated persistence in following up to ensure case resolution.
- Strong communication skills with the ability to explain complex clinical information.
Responsibilities
- Serve as a primary liaison between patients, providers, and pharmacies to ensure medication regimen continuity.
- Manage a queue of active clinical cases from initiation to final resolution.
- Review clinical cases to identify and address medication-related issues.
- Engage prescriber offices and patients via high-volume phone outreach.
- Document all encounters and case activities accurately in a case management system.
- Coordinate with pharmacies and payers to resolve medication access and optimization issues.
