Requirements
- Master's degree in psychology, social work, or related behavioral health discipline from an accredited institution.
- Proficiency in working remotely using technology.
- Strong proficiency in Google Suite (Calendar, Docs, Sheets, Slides).
- Proven experience with electronic medical records (EMR).
- Strong documentation and data entry skills.
- Demonstrated ability to perform motivational interviewing, behavioral activation, and problem-solving therapy.
- Ability to assess patient risk and formulate safety plans.
- Excellent interpersonal skills for relating to patients, families, and healthcare agencies.
- Clear verbal and written communication skills.
Responsibilities
- Accept patient handoffs and provide seamless care continuity.
- Develop rapport with patients to create a safe therapeutic environment and assist with self-insight.
- Guide patients in developing skills to manage triggers, prevent relapse, and reduce risk.
- Educate patients on medications, monitor adherence, and coordinate with PCPs and psychiatrists.
- Maintain daily documentation in the electronic medical record (MMR) within 24 hours of activity.
- Conduct risk assessments, develop safety plans, and perform crisis interventions as needed.
- Monitor and record patient progress monthly using validated assessments.
- Prepare and maintain diagnostic records and reports while ensuring patient confidentiality.
