Requirements
- Background as a Medical Assistant (MA), Certified Nursing Assistant (CNA), or similar healthcare role.
- Experience in care coordination, patient engagement, or remote patient monitoring (RPM).
- Proficiency with digital health tools, EHR systems, and telehealth platforms.
- Strong communication and interpersonal skills, with a patient-first mindset.
- Ability to work independently in a remote environment while maintaining productivity.
- Comfortable with structured but flexible scheduling, ensuring full-time work commitment of 40 hours per week.
- Bilingual (Spanish) proficiency is a plus.
Responsibilities
- Engage with patients through in-app messaging, phone calls, and telehealth check-ins to support adherence to their care plans.
- Monitor patient data (such as blood pressure readings) and conduct follow-ups with patients who have not submitted readings in several days.
- Educate patients on how to use digital health tools and properly measure vital signs.
- Escalate concerns related to abnormal patient readings or reported symptoms to RN Care Managers or clinical teams based on established protocols.
- Provide care coordination support, including appointment scheduling and answering routine patient questions.
- Assist in patient onboarding for Babyscripts' RPM program, ensuring patients understand expectations and have access to necessary resources.
- Document all interactions accurately in Babyscripts' care management platform.
