Requirements
- Active, unrestricted ACSW, LCSW, LMFT, or LPCC license in California
- 1+ year of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
- Working knowledge of government and community resources related to social determinants of health
- Ability to work Monday-Friday 8:30am-5:00pm Pacific Time
- General computer skills and working knowledge of Google Workspace and MS Office
- Excellent oral and written communication skills
- Positive interpersonal skills
- Bilingual (English/Spanish) skills are highly preferred
Responsibilities
- Assess member needs across physical, mental, palliative, and social domains
- Oversee the development of client care plans and goal setting
- Connect clients to necessary social services and community supports
- Advocate for clients with healthcare professionals
- Apply evidence-based practices like Motivational Interviewing and Harm Reduction
- Conduct outreach and engagement activities and log data in the CRM system
- Maintain patient health records in the Electronic Medical Record (EMR) system
- Complete monthly reporting for program compliance
