Requirements
- Active, unrestricted RN license.
- Ability to obtain California licensure within 6 months of hire.
- 2 years of people management experience (4 years preferred).
- 2-3 years of care management experience.
- 1-2 years of post-acute experience.
- Proficiency in Microsoft and Google applications.
- Excellent communication, motivational, and conflict resolution skills.
- Strong organizational and independent working skills.
- Proven experience in change management in rapid-growth environments.
- Must be based in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, or Washington.
- Ability to work Monday through Friday, 8:30am-5:00pm Pacific Time.
Responsibilities
- Supervise a team of 10-15 staff to ensure high-quality enrollments and care management services.
- Oversee client outreach, enrollments, and the creation of Individual Care Plans.
- Drive the team to meet key performance indicators (KPIs).
- Collaborate with the Director(s) of Enhanced Care Management to implement process improvements.
- Provide clinical oversight, including staff supervision, clinical sign-offs, and leading IDT meetings.
- Serve as a clinical consultant for complex cases and provide direct care management when needed.
