Requirements

  • Active, unrestricted LCSW or LMFT license in California.
  • Willingness to obtain additional state licensure as needed.
  • 2 years of people management experience (4 years preferred).
  • 2-3 years of care management experience.
  • 1-2 years of post-acute experience.
  • Proficient in Microsoft and Google applications.
  • Strong organizational and independent working skills.
  • Proven experience in change management in rapid-growth environments.
  • Excellent communication, motivational, and conflict resolution skills.
  • Working knowledge of Lean Six Sigma methodology preferred.
  • Must be based in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, or Washington.
  • Availability to work Monday through Friday, 8:30am-5:00pm Pacific Time.

Responsibilities

  • Supervise a team of approximately 10-15 direct reports within the ECM team to ensure high-quality enrollments and care management services.
  • Oversee client outreach, enrollments, and the creation of Individual Care Plans.
  • Serve as an escalation point for complex client needs and lead IDT meetings.
  • Drive team performance to meet key performance indicators (KPIs).
  • Provide clinical oversight approximately 30% of the time, including clinical sign-offs and supervision.
  • Collaborate with the Director(s) of Enhanced Care Management to implement process improvements.
  • Provide direct care management as needed to support organizational goals.
