- 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree; 3 years of experience in case management preferred.

Requirements
- Active, unrestricted RN license.
- Bachelor’s degree (or 6+ years of RN experience with 3+ years of case management experience).
- 3 years of experience in case management (preferred).
- Experience in a dialysis or transplant center (preferred).
- Experience with Medicare and Medicaid (preferred).
- CCM certification (preferred).
- Strong verbal and written communication skills.
- Ability to succeed in a challenging environment with changing priorities.
- Proficiency in Excel, OneNote, Outlook, and Word.
Responsibilities
- Handle inbound and outbound calls to deliver service to members.
- Educate members on kidney health, co-morbid conditions, and renal replacement therapy options.
- Engage members into the Kidney Health Program.
- Follow up with members based on complexity and policy cadence.
- Act as a patient advocate to resolve concerns and barriers.
- Coordinate care as a liaison between patients, support networks, physicians, and ancillary providers.
- Utilize community resources and programs for effective care planning.
- Navigate technical applications including Excel, OneNote, Outlook, and Word.
