Requirements
- Active, unrestricted RN license.
- Bachelor's degree (or 6+ years of RN experience including 3+ years in case management).
- Three years of experience in case management preferred.
- Experience in a dialysis center or transplant center preferred.
- Experience with Medicare and Medicaid preferred.
- CCM (Certified Case Manager) certification preferred.
- Proficiency with Excel, OneNote, Outlook, and Word.
- Ability to succeed in a challenging environment with changing priorities.
- Strong verbal and written communication skills.
- Strategic thinking and planning capabilities.
Responsibilities
- Handle inbound and outbound calls delivering service to members.
- Educate members on kidney health, co-morbid conditions, and renal replacement therapies including dialysis and transplant options.
- Engage members in the Kidney Health Program and perform follow-up based on patient complexity.
- Serve as a patient advocate to resolve concerns or barriers to care.
- Coordinate care as a liaison between patients, support networks, physicians, and providers.
- Utilize community resources and programs in care planning.
- Notify providers of identified patient needs and ensure compliance with HIPAA and operational policies.
- Navigate technical applications including Excel, OneNote, Outlook, and Word.
