Requirements
- Bachelor’s degree in Social Work, Public Health, Healthcare Administration, or similar.
- 4-6+ years of healthcare experience, specifically in case management, crisis intervention, or coordination.
- Fluent in Haitian Creole and English.
- Experience developing and facilitating care plans.
- Ability to manage up to 40 care projects simultaneously with efficiency and compassion.
- Strong ability to navigate the healthcare sector and communicate in an empathetic, clear, and thorough manner.
- Proficiency with macOS, Zoom, GSuite, Slack, and 1Password preferred.
- Must have a private, dedicated, and distraction-free workspace.
- Strong internet connection and secure, private phone access.
Responsibilities
- Take ownership of the member-facing relationship, ensuring high-quality, timely, and thorough execution of priorities.
- Proactively identify opportunities for process improvements that enhance care delivery, efficiency, and member outcomes.
- Coordinate care, address complex caregiving needs, and develop tailored solutions to support families.
- Document and share caregiving resources to enhance team effectiveness.
- Collaborate closely with Care Advisers to ensure seamless execution of member tasks.
- Provide mentorship and guidance to team members by sharing insights and best practices.
- Represent Wellthy externally at events such as health fairs, webinars, or open enrollments.
