Requirements
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.
- 2 – 4 years of related experience.
- Active RN (Registered Nurse) - State Licensure and/or Compact State Licensure.
- Pediatrics or foster care experience is preferred.
- Ability to develop and assess personalized care plans.
- Proficiency in assessing social determinants of health and barriers to care.
- Strong collaboration skills to work with members, families, and healthcare provider teams.
- Knowledge of disease processes and healthcare benefit navigation.
Responsibilities
- Develop, assess, and facilitate complex care management activities to provide high quality, cost-effective healthcare outcomes.
- Evaluate member needs, barriers to care, and social determinants of health to recommend and facilitate care plans.
- Collaborate with providers, specialists, and community resources to identify and address member unmet needs.
- Provide ongoing monitoring of member status and progress, revising care plans as necessary.
- Facilitate timely access to needed care and services through collaboration with the provider team.
- Provide education to members and caregivers regarding disease processes, care options, and healthcare benefits.
- Maintain accurate documentation of all member information and care activities to ensure regulatory compliance.
