Requirements
- 3 years of experience in health care coordination, community health work, or similar healthcare field.
- Experience with a team-based approach to care.
- Excellent working knowledge of community resources.
- Exceptional ability to prioritize and task-switch.
- Strong communication and teaching skills.
- Intermediate proficiency with MS Office Suite, including Outlook and Teams.
- Ability to work in a dedicated, private space for PHI/HIPAA compliance.
- Home internet with minimum 25 Mbps download and 10 Mbps upload speed.
- Self-directed and motivated to resolve issues professionally and creatively.
Responsibilities
- Perform telephonic outreach to enroll and engage patients.
- Develop and implement care plans addressing unique patient needs under NCM direction.
- Facilitate seamless transitions of care across care settings and dialysis centers.
- Conduct thorough patient assessments to identify social, economic, and environmental factors impacting health.
- Connect members to community resources such as housing, transportation, and food assistance.
- Advocate for members by identifying barriers to care and addressing health inequities.
- Maintain communication with patients, families, and healthcare teams.
- Ensure accurate documentation of interactions, assessments, and care plans.
