Requirements
- Active RN license in good standing.
- 3+ years of clinical practice in an intensive care unit, hospice, or palliative care role.
- Proven experience facilitating goals of care and end-of-life decision discussions.
- Intermediate proficiency with MS Office Suite, specifically Outlook and Teams.
- Ability to work in a fast-paced, evolving startup environment.
- Secure, dedicated workspace for PHI/HIPAA compliance.
- Reliable high-speed internet (min 25 Mbps download / 10 Mbps upload).
- Current knowledge and competency in hospice and palliative care nursing.
- CHPN (Certified Hospice and Palliative Care Nurse) preferred.
- BSN preferred.
- 1+ years of utilization management experience preferred.
- Compact RN licensure preferred.
Responsibilities
- Build empathetic relationships with patients and families.
- Educate patients and families on diagnosis, prognosis, and treatment options.
- Complete advance care planning and facilitate Goals of Care (GOC) discussions.
- Translate patient values and goals into clinical care plans.
- Coordinate care across multiple disciplines including home health, palliative care, and hospice.
- Analyze patient data and monitor progress towards outcomes.
- Engage internal and external resources to address social determinants of health.
- Educate internal and community partners on hospice and palliative care referral pathways.
- Maintain accurate clinical documentation and records.
- Participate in interdisciplinary team (IDT) meetings and case conferences.
