Requirements
- 2+ years of experience as a Health Coach.
- Associate's degree (Required).
- Must be able to obtain a direct/hardwired internet connection to a modem/router (WiFi/satellite not permitted).
- Internet speed: Minimum 25 Mbps download and 3 Mbps upload.
- Ability to maintain a quiet, secure, private, and HIPAA-compliant home office.
- Proficiency in Microsoft Office 365 (Teams, Outlook, Word, Excel, PowerPoint).
- Experience navigating secure systems like VPNs and EHR portals.
- Comfortable troubleshooting basic technical issues independently.
- Commitment to working within the state and city of residence.
- Health Coach Certification (Preferred).
- Experience in care management, healthcare, social work, or community/public health (Preferred).
- Managed Care/Medicare experience (Preferred).
Responsibilities
- Instruct group coaching classes addressing common health concerns for members with chronic conditions.
- Provide information, referrals, and support to Medicare members regarding health resources and plan benefits.
- Educate members on topics such as medicine management, nutrition, safety, and mindfulness.
- Assist members in coordinating with healthcare providers and Aetna Care Management teams.
- Document all member interactions accurately in the designated member records.
- Demonstrate empathy while troubleshooting resource availability and closing member gaps.
- Follow program workflows and conduct follow-ups to review personal action plans.
