Requirements
- 3+ years of relevant community outreach, facilitation, or healthcare experience.
- Community Health Worker (CHW) training or certification preferred.
- High school diploma or GED required; college degree preferred.
- Strong computer skills and ability to navigate web-based and app-based systems.
- Deep understanding of socio-economic and public health challenges facing disenfranchised populations in New Jersey.
- Excellent written and verbal communication skills.
- Valid driver’s license and reliable transportation.
- Ability to carry up to 30 pounds of supplies and stand/walk for duration of home visits.
- Willingness to complete and maintain all CMS-mandated training and state-applicable requirements.
- Flexible schedule for occasional work outside regular business hours.
- Must be ready for pre-employment substance abuse testing and background checks.
Responsibilities
- Execute person-centered care plans for patients with high-risk illnesses and monitor disease-specific treatment regimens.
- Act as a liaison between patients and billing practitioners, managing provider scheduling and post-discharge support.
- Conduct person-centered assessments to identify unmet social needs such as food or housing instability.
- Teach self-advocacy skills and help patients navigate the healthcare landscape and access community resources.
- Support patient communication through templates and automation, managing data entry and troubleshooting in the Case Management System.
- Maintain detailed documentation of all patient interactions and activities according to CMS compliance standards.
- Use motivational interviewing to assist patients in managing the stress of chronic illness and improving treatment adherence.
