Requirements
- Graduate of an Accredited School of Nursing.
- Hold an unencumbered RN license.
- Ability to work assigned shifts, including days, evenings, nights, weekends, holidays, and overtime.
- Commitment to work every other weekend (16 hours) plus additional shifts during the week.
- Nursing knowledge and skills to treat frail, elderly participants and manage complex clinical situations.
- Experience in triaging is ideal.
- Strong organizational and decision-making skills, especially in emergency situations.
- Self-directed and able to execute tasks in a changing environment.
- Excellent communication skills for interacting with seniors, families, and interdisciplinary team members.
Responsibilities
- Handle inbound and outbound calls, responding within scope of practice and consulting with on-call clinicians.
- Coordinate telehealth meetings between participants and clinicians.
- Ensure timely care delivery and resolve basic logistical issues, escalating to management as needed.
- Troubleshoot care delivery issues related to transportation, medication delivery, and hospital discharge.
- Support clinical care and home health teams in managing transitions between hospitals, skilled nursing facilities, and other settings.
