Requirements
- Bachelor’s Degree in Nutrition, Dietetics, or a related field from an accredited institution.
- Active unrestricted Registered Dietitian certification issued by the Commission on Dietetic Registration.
- Minimum of relevant state dietitian licensure or registration.
- Willingness to obtain and maintain additional state dietitian licensures as needed.
- 2+ years of experience in the delivery of care to patients with kidney disease or other chronic diseases.
- Knowledge of healthcare regulations including HIPAA.
- Proficiency in using virtual care platforms for education, documentation, and team collaboration.
- Experience in patient counseling, education, and addressing sensitive health concerns.
- Ability to develop individualized nutrition guidance that supports self-management and sustainable behavior change.
- Strong communication skills to translate complex concepts into clear, practical, and empathetic guidance.
Responsibilities
- Collaborate with interdisciplinary team members, including nurses, social workers, and care coordinators, to deliver virtual care services.
- Conduct virtual nutritional assessments for patients with chronic kidney disease (CKD) and other co-morbid conditions.
- Develop personalized, evidence-based nutrition care plans and calculate nutrition requirements tailored to patient needs.
- Educate patients and their caregivers on kidney-related dietary and lifestyle adjustments.
- Maintain thorough and timely documentation of patient interactions, care plans, and interventions.
- Practice within the registered dietitian scope of practice and make interdisciplinary referrals as needed.
