- A minimum of two (2) years of experience working as a Registered Nurse, including Hospice experience
Requirements
- Associate degree or higher from an accredited School of Nursing.
- Active Registered Nurse (RN) license in the state of residence.
- Minimum of two years of experience as a Registered Nurse.
- Prior hospice nursing experience.
- Demonstrated proficiency with tablets, mobile phones, and EMR software.
- Ability to work assigned hours: 12A-8A Monday – Friday, and every other weekend 8PM-8AM.
Responsibilities
- Manage all triage calls during assigned business hours.
- Report information to the team at the end of shifts and sync electronic documentation in a timely manner.
- Respond to patient and family calls in a professional manner, addressing individual, cultural, and spiritual differences.
- Make patient care decisions including giving advice, contacting physicians, or scheduling visits.
- Triage after-hours calls to ensure patient safety, symptom management, and reduction of preventable hospital admissions.
- Coordinate after-hours care, referrals, staffing, and scheduling of visits or supplies.
- Review after-hours referrals and coordinate communication across disciplines and regional staff.
- Document all patient calls and encounters in real time while maintaining accurate records.
