- ADN or BSN degree.
- 5+ years of clinical experience, preferably in family medicine.
- Strong understanding of triage protocols and patient assessment.
- Proficiency in clinical documentation.
- Experience in care management or chronic condition support (desired).
- Experience in triage nursing (desired).
- Current licensure in CA, NY, MI, MA, or MN (desired).
Responsibilities
- Perform clinical triage to assess patient health concerns using evidence-based protocols.
- Provide clinical navigation support including condition education, care planning, and medication adherence.
- Leverage input from the clinical team to determine optimal care options for complex patient needs.
- Document symptoms, assessments, guidance provided, and member responses accurately.
- Engage in proactive outreach for high-risk and rising-risk members to support lifestyle and behavior changes.
- Coordinate screening services and follow-up for high-risk patients.
- Maintain a patient-centric, professional attitude as a trusted advisor.
