Requirements
- Current, active unrestricted Tennessee license in Nursing (RN) or behavioral health field (Master's level or above such as Ph.D., LCSW/LMSW, LLP, MHC, LPC).
- RNs may hold a license in their state of residence if participating in the Nurse Licensure Compact Law.
- 3 years of clinical behavioral health or substance use disorder experience.
- 1 year of experience knowledgeable about community care resources and levels of behavioral health care.
- Proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint).
- Strong organizational, critical thinking, problem-solving, and analytical skills.
- Excellent oral and written communication skills.
- Experience with Motivational Interviewing Techniques and Adult Learning Styles.
- Ability to work independently in a fully remote setting.
- Positive relationship building skills and ability to engage and motivate diverse populations.
Responsibilities
- Manage a mixed caseload consisting of both ongoing members and short-term cases.
- Provide comprehensive care coordination and case management for dual-eligible members with complex behavioral health needs.
- Collaborate within an integrated care team to align behavioral and physical health services.
- Support utilization management functions for complex and non-routine cases.
- Serve as a liaison between members, providers, and internal/external customers.
- Perform case management activities including assessment, planning, implementation, monitoring, and evaluation.
- Perform case management activities in community settings including face-to-face member interactions as required.
