Requirements
- Master’s degree in Social Work (MSW) from an accredited program.
- Active LCSW licensure in one or more U.S. states.
- Minimum of 5 years of experience working with patients affected by cancer or complex chronic illness.
- OSW-C certification preferred.
- Strong experience in delivering therapy and psychosocial support using evidence-based modalities.
- Excellent written and verbal communication skills.
- Comfortable working in a fully remote telehealth environment.
- Tech-savvy with experience using EMRs and G-Suite.
- AdvancedMD experience is a plus.
- Ability to work independently while contributing effectively to a multidisciplinary team.
Responsibilities
- Deliver individualized clinical social work services to patients with cancer through evidence-based interventions.
- Conduct comprehensive biopsychosocial assessments to evaluate patient needs, strengths, and care goals.
- Provide evidence-based individual therapy and psychosocial support to patients and their families.
- Collaborate with multidisciplinary clinical teams to develop and implement integrated, patient-centered care plans.
- Maintain accurate and timely documentation in electronic medical records (EMR) and track patient progress.
- Ensure compliance with HIPAA and other healthcare privacy and regulatory standards.
- Participate in coordinated care planning to optimize patient quality of life and long-term survivorship outcomes.
