Requirements
- Licensed RN, Physical Therapist, Occupational Therapist, Speech Therapist, or Social Worker.
- Associate Degree required, Bachelor Degree preferred.
- Minimum of one (1) year of experience in an acute care setting.
- Minimum of five (5) years of experience in a customer service capacity.
- One (1) year of experience in home health preferred.
Responsibilities
- Obtain complete and accurate demographic information and medical history.
- Introduce services to patients/caregivers and coordinate start of care visits.
- Order requested DME, infusion services, and wound care.
- Perform community education to physician offices and facilities.
- Provide patient education at bedside and arrange follow-up appointments.
- Attend discharge planning meetings to ensure safe discharge home.
- Determine appropriateness of skilled home care/hospice services.
- Conduct follow-up customer service calls to patients/caregivers.
- Communicate with clinical teams regarding complex or chronic patients.
- Complete documentation in EMR and assist in obtaining regulatory documentation.
