Responsibilities
- Accept transfers from the Patient Enrollment team to conduct preliminary health assessments.
- Resolve patients' questions and create open dialogue.
- Identify and coordinate community resources for patients.
- Provide patient education and health literacy on chronic condition management.
- Assist with medication management, adherence, and refills.
- Ensure timely delivery of services such as Home Health, DME, and Home Infusion.
