Responsibilities
- Collaborate with a dedicated team, supporting a substantial patient panel alongside Primary Care Providers (PCPs).
- Triage inbound communications, including calls, faxes, email and instant messages.
- Act as a support system and escalation point for LVN/LPNs.
- Engage in comprehensive chart preparation and provide patient education.
- Oversee post-appointment processes, including diagnostic orders, referrals, and medication orders.
- Participate in daily huddles.
- Conduct virtual patient assessments, developing personalized care plans.
- Serve as a pivotal communication link among patients, PCPs, and specialists.
- Promote patient health through education on preventive care.
- Participate in crisis intervention and palliative care discussions.
