Requirements

  • At least 2 years practicing advanced diabetes management
  • Experience with CGMs, insulin pumps, and complex insulin regimens
  • Ability to sign a collaboration agreement with an Endocrinologist
  • Demonstrated competency in managing patients with complex co-morbidities (CKD, ESRD, CAD/CV)
  • Experience delivering telemedicine care
  • Excellent communication and interpersonal skills
  • OMA certification (CAEOM) preferred
  • Adherence to evidence-based treatments and pharmacotherapy

Responsibilities

  • Provide direct patient care in a telemedicine clinical setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management.
  • Collaborate with Endocrinologist colleagues and Registered Dietitians to co-manage a shared patient panel.
  • Review charts and conduct return visits for patients to ensure quality and continuity of care.
  • Provide advanced treatment planning and medical management support for complex endocrinology cases.
  • Prescribe non-controlled medications for obesity and diabetes.
  • Conduct weekly collaborative meetings and asynchronous communication with collaborating physicians.
  • Ensure clinical documentation and treatment pathways are followed consistently.
  • Assist in the professional development of colleagues and support continuing education.
  • Participate in team-based care initiatives and quality improvement efforts.
