- Board-Certified in Family Medicine
- Active IMLC compact license or residing in an IMLC member state
- 3+ years of post-residency work experience
- Experience with virtual visits/telemedicine
- Primary care and behavioral health experience
- Demonstrated experience in Acute Care or Urgent Care settings
- Prior experience with EPIC EMR
- Currently enrolled, or eligible for enrollment, as a Medicare provider
- Willing to commit 40 hours per week
- Extensive experience handling a high patient volume (3+ cases per hour)
Responsibilities
- Provide high-quality preventative, acute, and chronic care in a fully-remote, virtual setting.
- Elevate remote care by providing compassionate and meaningful patient encounters.
- Manage a dedicated panel of patients as their primary care physician.
- Co-manage patients with their assigned primary care physician and external partners.
- Educate patients on appropriate treatments and care plans for their health needs.
- Prescribe and refill medication as appropriate.
- Manage patient follow-ups for chronic care programs.
- Refer patients to specialists or acute care services when clinically indicated.
- Work on population health initiatives for risk-attributed patients.
- Support patients immediately post-discharge to improve understanding of treatment plans.
