Requirements
- Active Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential.
- 1+ years of patient-facing healthcare experience in ambulatory, primary, senior, or case management settings.
- If located in Texas, must hold a current, active CHW certification.
- Demonstrated ability to build trust and communicate with older adults and individuals with chronic conditions.
- Strong understanding of care coordination principles.
- Proficient with EHR systems, care management platforms, and digital communication tools.
- Self-directed and metric-aware.
- High school diploma or equivalent (Associate's or Bachelor's preferred).
- Proven remote work capability and reliable home office setup.
Responsibilities
- Serve as the primary point of contact for enrolled patients, building trust and rapport.
- Conduct regular check-ins to assess health status, care needs, and social barriers.
- Maintain a caseload of approximately 150 patients.
- Navigate patients through the healthcare system, coordinating appointments and facilitating provider communication.
- Conduct SDOH screenings to identify barriers like food insecurity and housing instability.
- Provide condition-specific education and coach patients on self-management strategies.
- Conduct timely follow-up calls within 24-72 hours of hospital discharge or ED visits.
- Document all interactions in real time, tracking time for CMS billing codes.
- Escalate clinical concerns to the supervising LVN promptly.
