Requirements
- High school diploma or equivalent
- 2+ years of experience in a virtual care, patient coordination, or healthcare support role
- Familiarity with virtual care workflows in a primary care or chronic care setting
- Prior experience supporting care in a multi-state, remote care delivery model
- Strong communication and organizational skills
- Comfort with remote tools, secure messaging, EMR platforms (Athena preferred)
- Tech-savvy, adaptable, and comfortable operating independently in a remote setting
- Knowledge of obesity medicine, weight management, or chronic care workflows (preferred)
- Familiarity with HIPAA-compliant telehealth platforms (preferred)
- Experience with Athena EHR (preferred)
Responsibilities
- Conduct remote patient check-ins and pre-visit preparation including medication reconciliation, health history intake, and screening questionnaires
- Document medical information accurately in the EMR and prep charts ahead of clinician visits
- Communicate with patients regarding upcoming appointments, test results, and follow-up instructions
- Support clinicians during virtual visits by entering real-time documentation, assisting with orders, and coordinating next steps
- Coordinate care: manage referrals, prior authorizations, and medication refill requests
- Ensure prompt and professional communication with patients, clinicians, and care team members
- Collaborate with the virtual care team to identify and improve workflows
- Maintain a warm, inclusive, and professional patient experience aligned with knownwell’s values
