Requirements
- Current, active, unrestricted LVN/LPN compact license.
- Active, unrestricted LVN license in the state of California.
- Minimum 2 years of hands-on nursing experience.
- Experience with patient phone triage (in-office or telemedicine).
- Proficiency in EMR systems and Google products.
- Comfortable using episodic vital sign monitoring devices.
- Ability to maintain a HIPAA compliant home work environment.
Responsibilities
- Monitor and screen incoming alerts, calls, and chat communications.
- Schedule and conduct audio-visual calls with patients.
- Report abnormal patient vitals or concerns to the Provider Team.
- Act as a patient liaison to ensure needs are met.
- Perform patient onboarding and program explanations.
- Register patients and document information in EHR systems.
- Review patient records and support holistic wellness.
- Troubleshoot technical issues with monitoring devices.
