  • Eligibility with either a National Board-Certified Health & Wellness Coach (NBC-HWC) certification or LPN+ medical licensure.
  • Robust knowledge of ADA Standards of Medical Care in Diabetes, AHA Hypertension Clinical Guidelines, and CDC DPP Lifestyle Management guidelines.
  • Substantial knowledge of the healthcare industry.
  • Motivated to learn necessary technology including computers and software applications.
  • Desire to excel in a performance-based environment, preferably with inbound/outbound telephonic components.
  • Ability to collaborate effectively in a team-oriented and fast-paced atmosphere.

  • Serve as an expert diabetes health coach to support individuals in achieving the best outcomes.
  • Engage with individuals empathetically and clearly, mainly via phone calls, with options for texts.
  • Deliver education and personalized guidance according to Perry Health protocols.
  • Recognize immediate care needs and triage to Perry Health clinicians as necessary.
  • Adhere to clinical quality policies, procedures, and legal compliance.
