Requirements:
Doctor of Pharmacy degree (PharmD) from an accredited school of pharmacy. Licensed pharmacist in CO. Certification in a relevant field of practice OR completed a postgraduate residency with at least 50% direct patient care experience OR one year of clinical services under a CPA/protocol. Strong verbal communication skills. Deep knowledge of pharmacology developments and FDA health regulations. Ability to manage processes, exhibit strong technical and clinical skills. Maintain constant focus on legal compliance. Highly productive with a focus on quality and details. Strong clinical knowledge with focus on disease state management and treatment guidelines. Responsibilities:
Provide medication management including reconciliation, dose optimization, and adherence counseling. Collaborate with the interdisciplinary care team to develop individualized care plans. Monitor medication regimens for clinical criteria, goals, and adverse effects. Identify opportunities for drug therapy optimization and cost savings. Participate in quality improvement initiatives and outcome studies. Optimize resources and workflows for prior authorizations and appeals. Collaborate with healthcare providers to ensure seamless medication management. Perform clinical assessments and educational counseling for patients. Ensure clinical criteria compliance with FDA, guidelines, and literature. Assist with workflow optimization initiatives. Collaborate with other departments to enhance productivity and quality. Advocate for patient safety and medication literacy. Maintain confidentiality of patient information. Update job knowledge through education and professional development. Attend team meetings and professional development activities. Stay updated with advancements in therapy management.
