Conduct intake assessments to evaluate Member readiness and determine appropriate level of care Facilitate group and one-on-one counseling sessions using Affect’s evidence-based curriculum Provide crisis support and coordinate appropriate referrals when needed Maintain timely, accurate documentation in our electronic medical record (EMR) system Collaborate with clinical supervisors on individualized treatment and discharge planning Ensure compliance with HIPAA and all relevant state regulations
