Telehealth Nurse Practitioner

2+ years as a Board-Certified Nurse Practitioner specializing in heart failure and/or internal medicine. Actively licensed in California, multiple state licenses strongly preferred. Willingness to obtain additional state licenses as needed. Fluency in English and Spanish is highly preferred. Previous experience overseeing complex chronic care programs strongly preferred. Previous telehealth experience strongly preferred. Willingness to work in a changing, dynamic environment. Ability to work from home in a HIPAA compliant environment is required. Motivated to provide health education and grounded in health promotion techniques. Ability to learn and proficiently use multiple EMRs and communication applications.

Virtually evaluate patients entering the program and create a care plan. Respond to alerts escalated from the nursing team. Independently manage changes in clinical status within scope of practice. Escalate out-of-scope clinical changes to lead MD. Liaise with the nursing team for medication, lab, and appointment coordination. Partner with the nursing team for medication reconciliation. Serve as the point of contact for patient primary care teams. Provide education to patients about healthy behaviors and self-management.
