Telehealth Nurse Practitioner

2+ years as a Board-Certified Nurse Practitioner specializing in heart failure and/or internal medicine. Actively licensed in California. Willingness to obtain additional state licenses. Fluency in English and Spanish is highly preferred. Previous experience overseeing complex chronic care programs strongly preferred. Previous telehealth experience strongly preferred. Willingness to work in a changing, dynamic environment. Ability to work from home in a HIPAA compliant environment.

Virtually evaluate patients and create care plans. Respond to alerts regarding patient vitals, physiology, and weight. Independently manage clinical status changes within scope. Escalate out-of-scope changes to lead MD. Liaise with nursing team for medication, labs, and appointments. Partner with nursing team for medication reconciliation. Serve as patient's primary care team contact. Provide patient education on healthy behaviors and self-management.
