Requirements
- Minimum of 1-3 years of experience in healthcare, community outreach, or a related field
- Experience in engaging with diverse communities and providing culturally sensitive communication
- Fluent in English and Spanish (verbal and written)
- Strong interpersonal and communication skills
- Detail-oriented with excellent organizational skills
- Comfortable with technology and proficient in using electronic health records (Athena experience preferred)
- Ability to work a flexible schedule, including evenings and weekends
- Access to reliable transportation
Responsibilities
- Conduct in-person outreach attempts
- Schedule intake appointments and assist with form completion
- Attend outreach events and represent Zócalo Health
- Build and maintain relationships with community partners
- Convert leads to patients
- Document all outreach attempts and outcomes
